Fill all forms out completely and bring them into our office, along with a photo ID and your insurance card(s), and please arrive 15 minutes prior to your visit.

If you prefer, you may fax the forms to our office at
(816) 455-8901, you can also scan and email them to checkin@finefootcarecenter.com.

Completing these steps ahead of time will allow us to verify your insurance benefits and will save you time on your initial visit!
